HISTORA announces that Dr. Elena López Alvar has assumed the role of Chief Executive Officer. Pedro Guitián, co-founder and MD, DDS, PhD, moves to President, continuing to shape the company's long-term clinical vision and strategy.

This is not a change in direction. It is an acceleration.
The clinician becomes the CEO
Elena's path to the CEO role is built on 15 years at the intersection of clinical practice, scientific research, and operational leadership.
She is a dentist and oral surgeon, a Doctor in Health Biotechnology from the Universidad de Santiago de Compostela, and the Deputy Clinical Director at Clínica Guitián. Her doctoral research produced evidence-based predictive models for peri-implant disease, integrating clinical, genetic, and microbiological factors — work recognized with the National Colgate Prize in Prevention and Periodontics. Alongside her research career, she has maintained active clinical practice, with a focus on oral rehabilitation and aesthetic dentistry.
She built HISTORA because she lived the problem every day. As a practicing clinician, she watched patients arrive at her clinic without their records. She watched colleagues burn time re-sending files. She watched DICOM data locked behind proprietary software that specialists couldn't open without expensive installations. She watched front-office staff lose hours every week to manual workflows that had no business existing in 2025.
She didn't want a better workaround. She wanted the problem structurally solved.
That conviction — rooted in 15 years of clinical reality — now drives HISTORA's strategy from the top. Elena brings clinical authority, research rigor, and the operational focus needed to translate that vision into a platform that works at scale.
"I spent years as the clinician who needed this platform to exist," said Dr. López Alvar. "Now I get to build it the right way — starting from what patients and professionals actually need, not from what's technically convenient."
Agentic-first: what it means for HISTORA
Under Elena's leadership, HISTORA is committing to an agentic-first product philosophy. Every workflow in the platform — from the moment a file is uploaded to the moment it reaches a specialist — is designed to be handled by AI agents, not manual steps.
In practice, this means:
- Automatic file organization. When a clinic uploads a CBCT scan or a set of periapical X-rays, AI agents classify by tooth region, record type, and patient history — without staff touching anything.
- Referral packets, assembled automatically. When a dentist refers a patient to a specialist, the agent gathers relevant imaging, notes, and history and compiles a complete packet — ready to send with a single click.
- Proactive history surfacing. Before an appointment, agents surface the patient's relevant history so the clinician walks in already informed.
- Completeness checks. If a record is missing, the agent flags it before it becomes a clinical problem.
The agent doesn't replace the clinician's judgment. It handles everything that doesn't require it — so the clinician can focus entirely on the part that does.
This philosophy comes directly from Elena's clinical experience: the administrative burden in dental practices isn't a minor inconvenience. It is a structural problem that consumes time, creates errors, and degrades patient care. Agentic AI is the only credible answer at scale.
Patient data is the center — not a byproduct
Elena's most fundamental conviction: dental data belongs to the patient.
Under HIPAA and GDPR, patients have the legal right to their records. In practice, exercising that right means calling a clinic, waiting days, paying copying fees, and receiving files that most software can't open. Most patients never try. This is the gap HISTORA was built to close.
The agentic platform treats patient data as the primary asset — not a secondary output of clinical workflows. The professional platform and the patient portal are not two separate products with a data bridge between them. They are two views of the same patient-centered data layer, with AI agents ensuring both sides always have what they need.
"The patient is the constant in dental care," Elena said. "They move between clinics, between cities, between countries. Their data needs to move with them — organized, accessible, and secure. That's not a feature. That's the product."
This applies equally to professionals. Dentists, specialists, oral surgeons, and lab technicians all work on the same patient. The intelligence layer that connects them — that surfaces the right record at the right moment — is exactly what AI agents are built to be.
A message from Pedro Guitián, President
Pedro Guitián, who moves to President, shared his perspective on what this moment means for dentistry:
"Dentistry is at an inflection point. For the first time, AI and structured dental data can do what no dentist or front-office team can do alone — connect every record, every image, every note into a coherent clinical picture that follows the patient wherever they go. The practices that understand this now will define what modern dentistry looks like in five years. Elena is the right person to lead that shift. She brings 15 years of clinical depth, a research career that demanded the same rigor we apply to our engineering, and the operational conviction to make transformation real — not theoretical. HISTORA under her leadership will be the platform that makes AI and data actionable for every dentist and every patient — not as a promise, but as a daily workflow."
What changes, and what doesn't
The clinical and engineering teams remain unchanged. Matías Molinas continues as CTO, bringing 27 years of engineering experience to HISTORA's DICOM rendering, secure data pipelines, and AI agent infrastructure.
What changes is the pace. Elena's mandate is to move HISTORA from a platform solving the dental data problem for early-adopter clinics to the infrastructure layer for dental data globally — the system every clinic and every patient can rely on, regardless of where they practice or receive care.
The agentic-first bet is the core of that plan. In a world where AI agents handle the coordination work, every clinic — regardless of size, software stack, or technical sophistication — can operate as if they have a dedicated clinical data team running in the background.
